Addendum 1 – October 5, 2006

 

 

The following information is provided in response to questions and inquiries received from interested contractors, as well as new information provided by the District.

 

 

Item 1:   The SOQ due date has been extended.  The SOQ is due on Thursday, October 26th, at 5:00pm at the address shown in the original document.  There will be a second mandatory Pre-Submittal conference on Thursday, October 12th, 2006, at 2:00pm.  If you attended the first mandatory Pre-Submittal conference, you will NOT be expected to attend.  There will be two additional assistance clinics held on October 17th and 19th at 10am.  All events will be held in the Swinerton Management and Consulting conference room, Building 6, 1700 W. Hillsdale Blvd., College of San Mateo,  San Mateo, CA 94402.

 

Item 2:   Under “General Conditions, Paragraph 2:  Explanations to the SOQ.,” replace:

Any explanation requested by an Applicant regarding the meaning or interpretation of this Request for Statements of Qualification must be requested in writing by 5:00 pm on Thursday, September 26, 2006.

With:

Any explanation requested by an Applicant regarding the meaning or interpretation of this Request for Statements of Qualification must be requested in writing (Fax and emailed questions are acceptable) by 5:00 pm on Friday, October 20, 2006    

 

Item 2:   Under “Instructions, item 1,” replace:

…must be received by mail, recognized carrier, or hand delivered not later than by 5:00 P. M. on Thursday, October 5, 2005.  Late submittals will not be accepted or considered.

With:

…must be received by mail, recognized carrier, or hand delivered not later than by 5:00pm on Thursday, October 26, 2005.  Late submittals will not be accepted or considered.

 

Item 3:   Under “Statements of Qualification Submittal Format,”  item 3.a.i.1:

Replace:

Insurance coverage amount(s) shall be no less than: Comprehensive General Liability - $5,000,000 for each occurrence - $5,000,000 general aggregate limit - $5,000,000 aggregate for products and completed operations.

With:

Insurance coverage amount(s) shall be no less than: Comprehensive General Liability - $1,000,000 for each occurrence - $2,000,000 general aggregate limit.  Delete “$5,000,000 aggregate for products and completed operations.”

 

Item 4:   Under “Statements of Qualification Submittal Format,”  item 3.a.ii.4:

Replace:

Deductible shall not exceed $10,000 and shall be the responsibility of the Submitter/Consultant.

With:

Deductible shall not exceed $20,000 and shall be the responsibility of the Submitter/Consultant.

 

Item 5:   Under “Statements of Qualification Submittal Format,”  item 3.c, “Fees:”

Replace:

Hourly rates should be based on the prevailing wage rates as established by the State of California.

With:

Hourly rates shall be provided.

 

Item 6:   Under “Statements of Qualification Submittal Format,”  item 3.c.3, “Fees:”

Add at end of paragraph:

 

If this calculation for unit prices does not apply to your profession, please indicate so.

 

Item 7:   Under “Statements of Qualification Submittal Format,”  item 3.a.iv.3,

Replace:

Said endorsement must name Marin Community College District as the additional insured.

With:

Said endorsement must name the San Mateo Community College District as the additional insured.

 

Item 8:   Change the Category heading in Exhibit A “Part C: Safety, Prevailing Wage, Disputes and Bonds,” to:

“Part C: Safety and Prevailing Wage”

 

Item 9:   Under Exhibit A “Part C: Safety, Prevailing Wage, Disputes and Bonds,” delete items 4 and 5.

              

Item 10: Under Exhibit A “Part E: Fee Schedule,” add the following paragraph:

“Design and professional service companies shall provide billing rates and reimbursement mark-ups, if any.  Other service firms shall use the following method to describe their fees (if applicable):”

 

Item 11: Exhibit F, item 1 “Insurance Information,”

Replace:

If Submitter has had their general liability insurance carrier for less than 5 years, please provide additional information below for balance of the last 3 years:

With:

Show insurance history for previous 5 years:

 

Item 12: Exhibit B, under “List Key Personnel that will be assigned to the Work on the District projects:” 

 

               Change “Supervisor” to “Project Architect or Engineer.”

 

Item 13: Exhibit B, under “Recent projects:” Add text shown in italics:

 

List three recent projects completed for an Educational or Public entity in the past five years and indicate who was the Construction Project Manager and Construction Superintendent. If a separate sheet is used, it must contain all of the following information:
