Forms
This page contains links to the most current and updated
Curriculum Forms that must be used when submitting new courses
and/or programs or modifying existing ones. You can be assured that
you have the latest form developed by the Committee by checking
back here frequently.
The
table below lists all the forms. The table includes the title of
each form and form guidelines.You must have Microsoft Word on your
computer to download these documents. Click
on the Form underlined link to access the form template. Word should
open and the form should display. You must save the form on your
local computer in a directory or folder of your choice.
All
new courses, course modifications, experimential courses, and technology
mediated courses must include a Course Outline
using the revised format. Refer to the Course Outline
form as well as to the instructional sheet Course
Outline Guidelines. When submitting your new course or
a modified course with anything but minor modifications to the Curriculum
Committee, you must e-mail the course outline file to Maria Norris
in the Vice President of Instruction Office. When submitting your
