Form Summary Table

Below is a summary of the forms to use when submitting curriculum.

Form D New Course| Form F Course Modification | Form X Experimental | Technology Mediated Form | Course Outline

Form D

If you are creating a new course you must complete Form D - New Course. If this course has a recommendation, prerequisite or corequisite, you must also complete the Content Review Form for Prerequisites, Corequisites and Recommendations. Refer to the informational sheet entitled Selected English Outcomes for Prerequisite, Corequisite and Recommended Preparation Content Review Forms prior to filling out the Content Review Form.

Additionally, all new courses must include a Course Outline using the revised format. Refer to the Course Outline form as well as to the instructional sheet Guidelines for Preparing a Course Outline. When submitting your new course or a modified course with anything but minor modifications to the Curriculum Committee, you must e-mail the course outline file to Maria Norris in the Vice President of Instruction Office.

Form F

Use Form F - Course Modification when modifying an existing course. Modifications may include any of the following: changes in prefix, number, title, method of grading, units and/or hours, number of repetitions, prerequisites, major change in course description. Remember, modified courses must include a new or revised Course Outline in the new format. 

If you modify or add a recommendation, prerequisite or corequisite, you must also complete the Content Review Form for Prerequisites, Corequisites and Recommendations. Refer to the informational sheet entitled Selected English Outcomes for Prerequisite, Corequisite and Recommended Preparation Content Review Forms before completing the Content Review Form. Refer to the Course Outline form as well as to the informational sheet, Guidelines for Preparing a Course Outline

When submitting your course outline to the Curriculum Committee, you are expected to e-mail the course outline file to Maria Norris in the Vice President of Instruction Office. 

Form X

Use Form X - Experimental Course Request when creating an experimental course, usually numbered 680 or 880. Experimental courses must include a Course Outline using the new format. Refer to the Course Outline form as well as to the informational sheet, Guidelines for Preparing a Course Outline. If you need further information about experimental courses, refer to the informational sheet called Experimental Course Guidelines which provides additional information about when and where an experimental course might be appropriate.

If the course has a recommendation, prerequisite or corequisite, you must also complete the Content Review Form for Prerequisites, Corequisites and Recommendations. Refer to the informational sheet entitled Selected English Outcomes for Prerequisite, Corequisite and Recommended Preparation Content Review Forms before completing the Content Review Form.

When submitting your course outline to the Curriculum Committee, you are expected to e-mail the course outline file to Maria Norris in the Vice President of Instruction Office. 

Technology Mediated Instruction

Use the Technology Mediated Instruction Form when you are creating an "on-line course" or a course where the majority of the class is taught through, but not limited to, the following technologies:

  • Audio technologies: radio, phone, voicemail, and audiocassettes
  • Video technologies: television, teleconferencing, prerecorded videocassettes
  • Information technologies: stand-alone work stations, CD ROM, prepackaged multimedia, e-mail, chatrooms and bulletin boards, and the World Wide Web.

Technology mediated courses must also include a Course Outline using the new format. Refer to the Course Outline Form as well as to the informational sheet, Guidelines for Preparing a Course Outline. If you need additional help in creating a technology mediated course, please consult with your Division Dean and the Vice President of Instruction.

If this course has a recommendation, prerequisite or corequisite, you must also complete the Content Review Form for Prerequisites, Corequisites and Recommendations. Refer to the informational sheet entitled Selected English Outcomes for Prerequisite, Corequisite and Recommended Preparation Content Review Forms before completing the Content Review Form.

Course Outline All new courses and modified courses with anything but minor modifications must include a new or revised Course Outline. This Course Outline must be in compliance with the standards specified in Sections 3 and 5 of the 1995 Curriculum Standards Handbook, Volume I: Legal and Procedural Requirements. Refer to Skyline's Course Outline Form as well as to our instructional sheet, Course Outline Guidelines.
