Planning & Construction:  Capital Improvement Program (CIP): Notice to Contractors, RFP's, RFQ's, Attendee Lists, Directions

In order to complete this application, you will need to open it using Microsoft Word® version 97 or later. There are three ways you can fill out the form:

1. Click here to use a form which you will print and fill-in by hand or with a typewriter. To use this form, choose the File->Print option to print the form.
2. Click here to use a form that permits you to enter the information directly into an on-line form.

Hints for completing the on-line form (Option 2)

• When you open the form, the first fill-in area (called a field) will be highlighted. You may just begin typing.
  
• Use the Tab key or the arrow keys to advance from field to field.
  
• You may press the space bar or click the mouse button to insert or remove the "x" in check boxes.
  
• You should view this form in Print view (View->Print Layout). You also may wish to view your document at 100% zoom in order to see it more clearly.
  
• Each fill-in area has a limited amount of space, you may need to attach additional pages and clearly cross reference each attachment.
  
• Print and save these documents as you would any Microsoft Word® document--be sure to use the Save As... feature and save the form to your own drive, otherwise the file could end up in your "temp" directory
